Thank you for your interest in our VR Loaner Program. To request one of the sets simply complete the form below and a staff member will email you to confirm your reservation. By submitting this form and receiving the equipment, you agree to:

• Read the Care and Usage Guidelines
• Be financially responsible for any lost or stolen while in your possession
• Return the equipment via UPS no later than the Saturday after your assigned week at your expense
• Email staff with the tracking number of the shipment
• Complete the User Survey  within SEVEN days, and
• Receive additional emails for educators about the Museum and its offerings
